Fee Payment Form Winter/Spring 2012Effective for January 2012 payments and beyond, this is the form we ask all members to use when they pay monthly or registration fees. This for may also be used to pay other special event (karate summer camps, martial arts /self defense / fitness workshops, etc) fees.

Belt Promotion Procedure - (Updated for 1/2011) This document provides a detailed description of how to apply for every belt promotion test, under Black Belt.

Belt Promotion Schedule – This is the most current schedule for Orange belt through Purple belt testing. Students should plan to complete belt promotion tests on the days and times set up on this schedule. testing outside of the dates and times specified on this schedule are subject to an additional testing fee. Brown Belt promotion tests are set up by appointment.

Belt Promotion Documentation – The following 3 documents are required to be completed and submitted to the head instructor at least 1 week prior to a scheduled belt promotion test.

Intent to Promote – This is part of the Belt Promotion documentation (1st of 3 documents) required for all school-age children who want to take a belt test. Parents and school teachers must approve the students request to take a belt test by signing off this form.

Belt Test Application – This is part of the Belt Promotion documentation (2nd of 3 documents) that must be completed by the student (parents may help) who wishes to take a belt promotion test. This is also a “report card” where the instructor records belt test comments for future reference.

Fitness Log - This is part of the Belt Promotion documentation (3rd of 3 documents). Students use this sheet to record the required physical fitness exercise repetitions necessary to accumulate for each belt promotion.
